Our Advanced Disease Management program offers specialized care for
people who are living with complex and serious illnesses.

At Atlas at Cedar Grove, this care is provided alongside curative treatments as an extra layer of physical, emotional, and social support.

Our comprehensive services include pain management, symptom relief, and educating patients and families about the disease’s progression.

In collaboration with Bridges Palliative Care, our skilled caregivers work closely with residents and their
 loved ones to create customized care plans focusing on comfort, emotional support, and quality of life.

Program features include:

 

  • Improvement in quality indicators
  • Reduction in rehospitalizations & unplanned transfers
  • Alleviates symptoms including side effects of treatment
  • Advanced care planning & goals of care discussions
  • Improving understanding of illness & its progression
  • Post-discharge patient monitoring by physician
  • Collaborative care with other physicians and care team
  • Appropriate at any stage of a progressive illness

This program is covered
under most insurances.

My experience at this facility was amazing from the time I entered until the time I left. Everyone from the front receptionist to the nursing staff, cna’s, physical therapist even maintenance, activity reps, cleaning staff were really helpful, kind, and encouraging to say the least. Abraham the Assistant Administrator was caring, friendly, and involved with all the residents and their need’s and cares.I would recommend this facility to anyone. I also made many friends with the residents and intend to go back to visit.
